We gladly accept patients transferring from other dental practices. We invite you to schedule an appointment and set up a date and time to visit our Bemidji dental office.

To make your first visit to our office even smoother, before you see us, print and fill out our Consent for Release of Information form (PDF). Alternatively, you may fill this form out in our office upon your visit.
